Neurodevelopmental pathway referrals

Once we have received the completed referral form, we will contact you and ask you to submit the pre-assessment supplementary forms. 

There are two forms to complete. Please send the pre-assessment supplementary forms to the child's parents/carers and school as appropriate. They should send completed questionnaires back to you to collate. Once all forms are complete, please send them to us via email: epunft.seechs.singlepointofaccess@nhs.net 

Important: please ensure the pre-assessment supplementary forms are submitted in a timely manner. We are unable to progress the child's assessment without this important information. 

Once the supplementary forms have been received, we will send you an email acknowledgement. We will be in touch with next steps (phase three) in due course. 

Supplementary forms

Pre-assessment supplementary form - to be completed by child's parent or carer. Use this link to download the form. 

Pre-assessment supplementary form - to be completed by child's school or pre-school. Use this link to download the form. This form should be completed by a teacher that knows the child well. If the child is home educated, please request that someone in a setting other than the home completed this referral form. This could be a tutor, club teacher or coach, youth worker, case manager or a teacher at a previous school setting.

Once the initial referral forms and pre-assessment forms have been received, we will ask you to complete and submit additional information forms. These will be relevant for the child's age and potential diagnosis. 

Please be advised that these forms may take 30 to 40 minutes to complete.

ASD Forms for children aged under 11 years (pre-school and primary school)

ASD Social Communication Questionnaire (4-11, parent/carer) to be completed by parent/carer of children aged 4 to 11 years. Use this link to download the questionnaire. 

ASD Social Communication Questionnaire (early years, school) to be completed by the pre-school, nursery or school of children pre-school age or in Key Stage 1. Use this link to download the questionnaire.

ASD Forms for children and young people aged 11 and over (secondary school)

ASD Social Communication Questionnaire (12 and over, parent/carer) to be completed by parent/carer of children in secondary school. Use this link to download the questionnaire.

ASD Social Communication Questionnaire (16 and over, young person) to be completed by young people aged 16 and over who have been accepted for ASD assessment. Use this link to download the questionnaire.

For secondary school aged students, the school must submit a recent progress report and any SEN Plans the young person has in place.
