Once the initial referral forms and pre-assessment forms have been received, we will ask you to complete and submit additional information forms. These will be relevant for the child's age and potential diagnosis.
Please be advised that these forms may take 30 to 40 minutes to complete.
Please send the questionnaires to the child's parents/carers and school as appropriate. They should send completed questionnaires back to you to collate. Once all questionnaires are complete, please send them to us via email: epunft.seechs.singlepointofaccess@nhs.net

For secondary school aged students, the school must submit a recent progress report and any SEN Plans the young person has in place.
